This is a Le Mat, a Civil War-era contraption in which a cap-and-ball cylinder with .36 to .42 caliber chambers revolves around a 16-gauge smoothbore barrel. The Confederates used them. Modern reproductions are available from Cabela's and Navy Arms. That rail -- if such it be -- does look damned odd!

Looks like a potmetal LeMat replica taht's been doctored up as an accessory to someone's Steampunk cosplay.
Actually, it looks pretty cool in a steampunk environtment. Outside of it, though, it looks more like a melon basher that a melon popper.
